Whether you're a family member, neighbour, GP practice, hospital team or social worker, referrals are quick, confidential, and we'll be in touch within one working day.
How referrals work
- 1Tell us about the person. Use the form opposite or call us directly on 01236 359027.
- 2We confirm consent. We'll contact the person (or their next of kin) to confirm they're happy to proceed.
- 3We arrange a visit. Free home consultation and matching with a suitable companion, usually within a week.
For professionals
We accept referrals from GP practices, district nursing teams, hospital discharge coordinators, third-sector organisations and local authority social work. Email referrals@niamz.co.uk for a referral pack.
